Prior to Surgery, a nurse from Clear Creek Surgery Center will call you at home. The purpose of this call is to obtain a brief medical history and baseline information about your health and medication usage so that we can take better care of you or your child. This call also gives you the opportunity to ask any questions you may have.
If you or your child have orders from your physician to obtain blood tests, EKG, or other tests or exams prior to your procedure, please arrange to have them completed prior to your appointment. The purpose of pre-admission testing is to gather information before surgery about your health status. Please bring the results of the testing with you or have the results faxed to Clear Creek Surgery Center at (303) 463-0929.
Please make arrangements for transportation home from the surgery center and for a responsible adult to be with you for 24 hours after surgery.
Preoperative Testing
Your surgeon will determine if any pre-operative testing will be necessary prior to your procedure. If testing is necessary your doctor will give you an order sheet that can be taken to a testing facility approved by your insurance company. The order sheet will alert the laboratory/radiology provider that you are scheduled to have a procedure here and the surgery center will receive the report. It is best if you have your pre-procedure tests completed at least three business days before your procedure.
Preoperative History On Line
You may access the preoperative history form online. After completing this form, a copy will be sent to Clear Creek Surgery Center for review. If there are any questions, a nurse will contact you by phone. A copy of your history will also be emailed to you for your records.
Preoperative Phone Call
A Registered Nurse from the Surgery Center will call you a day or two prior to surgery to collect information on your medical history and to provide you with pre-surgical instructions. Please call us at (303) 422-5555 if our nurse has not contacted you.
